The nurse who came to give me my vaccine did not ask my name and date of birth or verify the medication in the EMR before administration. Otherwise I have great things to say about Dr. Cox. She thoroughly went through my medical history, provided sound guidance, explained her rationale, and made me feel like I was participating in my own healthcare. I am glad I have had the opportunity to establish care with her.

Be on time for appointment - I was early, then was sent to a room and then waited for almost 45 minutes after appointed time for doctor to be seen.

Nothing really. It was a great experience. Saw me on time and listened to everything I said and documented it. Very pleasant nurse and Dr. Cox. Definitely recommended her to others.

It would be useful to add to the appointment reminders a statement as to whether FASTING is required for that particular appointment. For this appointment, I went without food for about 15 hours because I expected to give blood samples. When I arrived I learned that the blood samples were not in the plan for that day. Alternatively, I can assume no fasting is required unless specified in the reminder and that is the position I will take unless directed otherwise.

I cannot think of anything. Very confident I am in good hands with my new doctor.

I just wanted to establish a new patient relationship with a doctor. Gosh, now you offer so many vaccines, and it can get overwhelming. I find it more comfortable in a holistic practice.

I met the doctor for the first time, she knew my long medical history. Also I commented to my wife as we were leaving it has been a very long time a doctor looked me in the eyes as we were talking, very impressive!

You don't have to do anything. People were great. She was great. They made me understand everything. They explained everything. They made me feel safe. Thank you.
